The applications should, interalia, contain the following information :

(a) Constitution of the applicant

(b) Business profile for past three years

(c) Sales tax registration number

(d) Details of agency/dealership for products and at least two references from customers, preferably, reputed organization

(e) Income tax clearance certificate
